In Australia, the Chronic Disease Management (CDM) program, formerly known as the Enhanced Primary Care (EPC) program, is designed to provide additional support and access to allied health services for individuals with chronic and complex health conditions. The condition also needs ongoing management and coordination between multiple healthcare providers.

The CDM treatment plan is prepared by your general practitioner (GP) or a medical specialist, detailing the specific allied health services needed. Once your plan is set, you can access the services listed, with part of the cost covered by Medicare.
